How to request records

Do you need a copy of your pets records?

We would be happy to help you with that! All of our records can be sent via email for your convenience. Please email [email protected] with your name, your pet's name, and what records you need.  If you would like us to share records with another facility, please include their contact information in your email. Please understand that it may take 2-3 business days to fulfill your records request.
